Customize your Companion by including your Intake Note. We understand that you've got your go-to questions and like the information they provide, so we want to give you the opportunity to have the simplest way to log all of your client's thoughtful information. 
 
When you complete your purchase, you will be given a form to include your intake. Please be thourough and write it how you'd like it to be structured. Once you're finished with the form and send it to YourEMDRGuide@gmail.com, our team will turn it into a fully fillable PDF that you can use as your intake note!
